Completion of this form authorises representatives of either Major Recruitment Limited, Or Leader Online to check your driving licence against the
records held by the Driver and Vehicle Licensing Agency (DVLA) as required during your
employment.
Please ensure that you supply all the information required below clearly, correctly. The form must be signed and dated. Any incomplete forms will be
returned to the sender and may result in the driving entitlement consent form being rejected.
You will then be asked to complete a new form and/or your entitlement to operate vehicles at work could be suspended.
